Welcome To Our Practice!
Our staff is
committed to the success of your medical treatment and care, and we want
to do our very best to exceed your every expectation. We are privileged
to have you as one of our patients and we look forward to your visit
with us. Please complete the following items prior to your initial
visit:
1. Complete the New Patient
Information form and
bring it with you for your visit.
2. Complete the
New Patient Questionnaire and
bring it with you for your visit.
3. Review the
list of Medications to Avoid and
if you are taking any medications on the list, stay off of them for the
number of days indicated. For the most part this includes antihistamines
and over-the-counter cough and cold remedies for at least seven (7)
days. These medications prohibit us from doing appropriate testing if
necessary.
4. Obtain a
referral for your visit, if required by your insurance company, by
contacting your primary care or referring physician. Have the referral
sent to our office or bring it with you the day of your appointment.
5. Ask your
referring physician and specialists to send any related medical records
or labs that have been performed.
6. Your visit is
comprehensive, so prepare to spend up to four (4) hours at our office.
The length of this visit also makes it difficult for younger children or
siblings to accompany parents so you may want to make other arrangements
for them.
7. Bring your
primary and secondary insurance card(s) and driver's license.
8. Come prepared
to pay your co-pay, deductible, and coinsurance as these are due at the
time of your visit and will be collected upon your arrival. We accept
cash, check, Visa, MasterCard and Discover. For patients with high
deductible plans, please come prepared to pay the first $200.00 of your
deductible, if not met, and we'll work with you to setup a payment plan
for the remaining balance. In the event that you do not have any
insurance coverage, please contact our office to discuss available
resources.
9. Review the

Our Practice Brochure . This brochure contains contact
information about our practice and how we care for our patients and it
summarizes some of our patient-care policies. An understanding of this
brochure will minimize any confusion between you and our office.
10. In the event
you may not be able to keep this appointment, please contact our office
at least five (5) days in advance. This will allow us to serve you and
other patients better.
